Songs on MP4 get mixed up?

While playing a certain song on my MP4 Player there's suddenly a scratchy sound and then another track starts playing even though the player shows the info of the track that was played initially. there is a specific time when that happens (when the second track 'takes-over') every time I try to play that song. is there a way to fix it?
